Choosing protection for your wireless phone means added security. What Wireless Phone Insurance means to you is fast, hassle free protection when you need it.
Wireless Phone Insurance protects your wireless phone against loss, theft, accidental physical or liquid damage, and mechanical and/or electrical failure after the manufacturer's warranty period has expired. The program has a monthly premium of $4.99 and a non-refundable deductible for each approved claim. The premium and deductible (for an approved claim) are added to your monthly billing statement.

Deductible: Effective Nov. 3, 2008 per the terms of your new Insurance Coverage Certificate, each replacement phone is subject to a $50 or $125 non-refundable deductible per approved claim depending on the phone model

Limits: Two claims within any consecutive 12 months, with a maximum replacement value of $1500 per claim

Claims may be fulfilled with new or refurbished replacement equipment and may be a different brand, model or color than the covered equipment. Compatibility with current accessories and features is not guaranteed.

Eligibility and Enrollment Options
You can enroll in Wireless Phone Insurance when you activate or upgrade your AT&T device. You must enroll within 30 days of activation or upgrade. Wireless Phone Insurance is not available for and does not apply to: Apple iPhone™ (all models), Phones on GoPhone® accounts, Laptop computers and Netbooks.
To enroll in the optional Wireless Phone Insurance program, visit your nearest AT&T Store representative call AT&T customer service at 1-800-331-0500.
You can also enroll for Wireless Phone Insurance when you activate new service or upgrade online. Simply select the option to enroll your mobile number in Wireless Phone Insurance from the "Add Services" pull down menu before finalizing your purchase.
Equipment Coverage
Once your mobile number is enrolled in the program, Wireless Phone Insurance provides coverage for your handset. If reported as part of a covered loss, the program also covers: a standard battery, standard battery charger, SIM card, and one accessory - subscriber may select a carrying case, automobile cigarette lighter adapter, or standard earpiece(excluding BLUETOOTH®).
Premium: The $4.99 monthly premium will appear on your monthly billing statement upon enrollment. You may cancel your optional insurance coverage at any time and receive a pro-rated refund of your unearned monthly premium.
Deductible: All losses incurred through November 2, 2008 will continue to be charged a $50 non-refundable deductible per approved claim, regardless of model. A tier deductible structure, per the terms of your new Insurance Coverage Certificate for any losses incurred on and after November 3, 2008, will apply as follows:
- Equipment Tier 1: phones will continue to incur a $50 non-refundable deductible.
- Equipment Tier 2: phones will be charged a $125 non-refundable deductible.

Easy Claim Process
Call 1-800-801-1101 to file a claim. Claims must be reported within 60 days of the date of loss. Claims are limited to two approved claims within any consecutive 12 months, with a maximum equipment value of $1500 per occurrence.
- If your phone was lost or stolen contact AT&T Customer Care at 1 800-331-0500 to temporarily suspend your service and prevent unauthorized use.
- Return damaged equipment by using the prepaid shipping label provided with your replacement equipment. Non-return charges may be added to your wireless bill for failure to return damaged equipment.
Replacement Equipment
Replacement equipment is shipped overnight once claim has been approved. The goal is to fulfill your claim with the same make and model, depending on availability. If the same model is not available, a similar make/model will be provided. Claims may be fulfilled with new or refurbished replacement equipment. Phone color, brand, model and features may be different and accessory compatibility is not guaranteed.
